This is a 5-month project supporting people who are clinically and extremely clinically vulnerable, living within Hart, who have become isolated due to the COVID 19 pandemic. A dedicated Rushmoor Healthy Living (RHL) colleague will be able to provide support for the people within the project, over the 5 hours a week dedicated to the project. The project is able to support up to 20 people over the 5 months.

The project will look to provide engagement, support, and assistance, both virtually and face to face, to enable the people to reconnect with others, their local and wider community, whilst improving their mental and physical wellbeing, increasing self-confidence for them to feel able and ready to return to their usual day to day activities outside of their homes. This will look to not only include the persons usual activities, such as shopping and picking up prescriptions but also encouragement/support them to engage with new activities such social, interest, and or exercise groups.

This project will support people to become more confident within themselves, their local and wider communities to be the able to reengage with their independent lives.
